During early development, John Wells and Noah Wyle originally envisioned the show as more a direct sequel to their previous hit NBC medical drama ER (1994), which Wells executive produced, wrote and directed. Wyle would have reprised his role Dr. John Carter. However, they were unable to secure the rights with original series creator Michael Crichton's estate and thus altered the concept and setting to make it more a spiritual sequel instead.
