It was a silent night in the middle of nowhere, a group of five friends, on vacation, decided to make a quick stop at an old house. Kiko, the funny and childish fellow who always played with his clackers; Alex, the bratty and flirty diva; Peter, the silent but friendly boy next door; Mia, the pure and kind-hearted woman and the love interest of Gio, the bad boy leader of the group, arrived at the old house that Peter had rented for them. Immediately, they were disappointed by how old and ugly it was. Due to Kiko's clumsiness, he tripped on the stairs while entering the house, but then he saw the old, grumpy female caretaker who signaled them to go inside. Confused, the group continued into the house, but Gio stopped Peter and scolded him for choosing the old house. Mia intervened, asking them to accept it as they had no other choice. Peter thanked Mia and went inside the house.

After some time, the old caretaker gave them a tour of the house, finishing by revealing the bathroom. Kiko stopped her and asked about the gate under the stairs. The old caretaker, with an angry tone, warned them not to go inside. Kiko was relieved, not wanting to go there, but his friends were scheming something for him.

They dispersed, and Gio opened a bottle of alcohol in front of Mia, making her angry that he started drinking again. Gio pleaded that he couldn't take it, and suddenly Alex arrived, understanding Gio's predicament, and took a sip from his bottle, making Mia even angrier and made her walked away. The group decided to play truth or dare. The bottle landed on Kiko, the group took advantage of the chance to dare him to go through the gate leading to the basement. Angry and annoyed, Kiko had no choice but to go through with it. While Kiko was gone, the group continued playing the game when suddenly there was a blackout, and they freaked out. Gio blamed their discomfort on Peter, who went away embarrassed to fix the electricity. As he walked away, Mia asked Peter to be careful, making Gio uneasy. Suddenly, Alex hugged Gio and flirtatiously admitted she was scared but prepared to use her pepper spray if someone did something stupid. An annoyed Mia walked away, and Gio chased her. While confronting Mia, a loud bang echoed, rattling them. Gio pulled up the flashlight on his phone, but a scared running Alex bumped into him, causing the phone to drop.

When he picked up the phone, he flashed a doll in front of them, shocking them. Alex pointed out that the doll was holding Kiko's clackers, suspecting it was Kiko's doing. Angry, they found the lights back on. Mia left to look for Peter, and Gio quickly followed her. When Alex started to leave, the clackers made a sound, making her curious. She looked at the doll and smacked it for scaring her. Peter was back with the group in the living room, talking about how they got played by Kiko with a doll. Peter, being cynical, asked where the doll was, but when Alex pointed it out, the doll was gone. Gio angrily shouted to Kiko to stop his shenanigans because it was late, and they needed to sleep. Everyone went to their own rooms except Gio, who wanted to take a bath first.

While taking a bath, Gio suddenly saw blood on his body causing him to panic. He quickly looked in the mirror, but there were no signs of blood on his body. Then, a loud scream from Alex echoed across the house. He and Peter ran to her room, but to their surprise, a scared and traumatized Mia was the only one in the room. Gio quickly consoled her while Peter continued looking for Alex, but with no luck. Both Gio and Peter suspected that maybe Alex and Kiko were in the basement. Peter gave water to Mia and Gio to help them relax, and after drinking, Gio confronted Peter about his red eyes. Peter reasoned that he was asleep when he heard Alex's scream. Gio decided it was best if they left the house and asked Peter to bring their things outside while he looked for Alex and Kiko. Mia quickly came to her senses and wanted to join Gio, and he agreed. When going down to the basement, Gio and Mia found a body wrapped in plastic. When he opened it, he found Kiko's dead and bloodied body. Both were surprised, and Mia stepped back, hitting another body - it was Alex.

The old caretaker then entered the room, making Gio realize it was all her doing. The old caretaker mumbled that the pure body of Mia was the only requirement left to finish her spell. Gio swore he wouldn't let her have her way, but suddenly both Mia and Gio lost consciousness due to the water that Peter gave them, mixed with poison. Gio fought his senses to not fall asleep, but the old caretaker smashed his head with a rock, killing him. Mia slowly regained consciousness but was still paralyzed. She was flustered when she looked around the area and saw a dead body of a girl, the doll, many symbols, and an altar with an image of Bakunawa, a deity of the underworld that eats the moon. Peter entered the room, and when Mia asked for his help, it was ignored. He asked his mom, the old caretaker, if the ritual to revive his daughter was complete. The old caretaker, now revealed to be a babaylan, confirmed that all preparations were now complete, and they could start the ritual. When the ritual was complete, the doll stood up, killing Mia in the process.
